snowman as patientIn an industry marked by a dwindling number of healthcare professionals charged to care for an exponentially increasing number of patients – each of us as precious and singular as snowflakes – the pressure on providers is acute, to say the least. Because all healthcare organizations – whether branded as ACOs or not – encompass a range of participants from hospitals and labs to home healthcare providers, they need to ensure each participant operates in a coordinated manner that reduces inefficiencies while improving the health of patient populations. The goal here is what’s referred to as “Patient 360” and to reach it providers need to leverage critical patient data stored across disparate information silos to enable a complete, actionable view of every patient.

Patient-focused providers are better equipped to minimize hospital readmissions and, more importantly, reduce the likelihood of catastrophic patient outcomes attributed to communication breakdown in the continuum of care. Achieving clear insight into a patient’s health history has obvious benefits for each of us as consumers, but the advantages to a healthcare system’s overall “health” and level of accountability are also remarkable in that they empower providers to reduce costs while advancing patient care.
